These physicals can be used
for preschool, Head Start, school, and athletics. Physicals are available
for children ages 1 month to 18 years. Medicaid enrolled children are
welcome.

Physicals Include
General assessment, lead
screenings (ages 1-6 years), height & weight, complete physical, vision
screening, hearing test, immunizations, nutritional education, and other
consultation as necessary.
